Kai Shin New Patient Registration
We currently outsource co-occurring mental health and Medication-Assisted Treatment (MAT) services to Kai-Shin. If you are experiencing withdrawal symptoms and wish to be seen sooner after admission, please complete the new patient registration form in advance. If not completed beforehand, a paper copy will be provided upon admission.
